  • We will provide you with a support system that operates 24 hours a day and 7 days a week; you can raise any of your concerns and questions.
  • We will be continuously assessing and reviewing your plan, including the progress that has been made to support your medications and routine appointments.


Chronic Care Management Services


At Choice Medical Group we follow strict standardized and robust CCM oversight policies.


  • We maintain an electronic copy of the care plan assigned to the patient by the provider. This copy is available to every individual that is involved in the Chronic Care Program.
  • We will be continuously assessing and reviewing your plan, including the progress that has been made to support your medications and routine appointments.
  • We work in tandem with your specialists and home health nurse to monitor the health of the patient as well as the progress.
  • We support the patient in having access to query resolution or urgent care needs through helplines functioning 24/7 by telephones, messages or e-mails. Routine appointments can also be booked through these helplines.
  • We review and assess the preventive care needs of the patient, medications, and reconciliations and ensure the care plan is updated.
  • We coordinate home services and transition plans between different service providers.
